Every Landing Page and form must be assigned to an existing CRM or Marketing List. The list assignment informs the system as to the final location where incoming conversions should be recorded. It also determines your field mapping options on the form building step, as you'll have the option to map a form field directly to a custom field on your assigned CRM or Marketing list.
Sync with CRM Campaign
Successful conversions can be appended to a campaign in your CRM. This can either be with an existing campaign or a new one can be created specifically for these conversions.

Every form will have a submit button associated with it. You can customize the label on a standard browser button or you can upload a custom image button. This is another area where you can reinforce the CTA (call-to-action) to your visitor, by customizing the button label to match your offering. For example, if your CTA is a downloadable white-paper, then you could label your button 'Download Now'.
Align Submit Button
Left, center, or right justify the submit button or image on your form.
On Submit Send Alerts To
Upon a successful form conversion, send an alert to an email address or addresses alerting the recipient to a recent conversion. Information submitted will be available in the alert.
On Successful Submission
What happens after your visitor converts on a landing page is a very important step. Are they going to be redirected to a thank you page? Will the successful conversion direct to a downloadable file? Or, will you just show a simple confirmation message? That choice is yours and there is lots of room for creative thinking with these three options.
Note: You can also create an Automated Campaign that deploys an email confirmation upon successful conversion (or even a series of emails), alerts a sales agent, schedules a sales call and much more.

Step #6. Build your form
Our drag and drop form builder makes it quick and easy to assemble a custom form to include on your landing page.
Adding Fields to your Form
Simply select a desired field from the left tool panel and drag it to the form canvas on the right preview column. You can position your field in any order you wish by simply dragging it up or down vertically.
Customizing your Fields
Once you add a field to the canvas, a field customization area will appear in the left tool panel. Each field contains its own customization parameters that are controlled on a field-by-field basis. These parameters include:
> Field Label
The field label serves as a title for your field and appears above each form field, above the optional field description.
> Map Field to
Form field values can be mapped to existing custom fields that are contained in the assigned CRM or Marketing List for your landing page. This is an optional parameter and any fields that are not mapped to a custom field are still stored and associated with the record that converted, but its not saved as an actionable custom field. Custom fields are very valuable for both Lead Scoring (Scoreboard) and Automated Campaigns. Since you're not restricted to any fixed number of custom fields, we recommend that all form fields be mapped to a custom field as a best practice.
> Field Description
The field description appears beneath the field label / title and above the actual form field. It's a great place to describe exactly what information you're requesting and why you're requesting it.
> Default Value
You can specify a default value to be shown in your form field. This can be helpful with establishing the format that you wish your visitor to follow or simply making it easier for your visitor to convert, if you know a certain value is most likely to be entered by the visitor anyway.
> Required Value
You can specify which form fields are optional and which are required. If a field is required, it must contain a value before the visitor will be able to successfully submit the form.
By selecting this option you limit the number of times the form can be submitted by an individual visitor or lead to one time.
Once you add a field to the canvas, a field customization area will appear in the left tool panel. Each field contains its own customization parameters that are controlled on a field-by-field basis.
> Name Field
The Name field is formatted for First and Last name. We recommend this field be used on all of your landing pages and forms.
> Email Address
An Email Address field is required for all landing page forms. We include the option of a 'Confirm Email Address' field that would require a visitor to enter their email address twice, to ensure data integrity and prevent invalid email addresses.
> Single Text Field
The Single Text field is best utilized for open-ended data collection in situations where you could have a wide variety of entries from visitors.
> Paragraph Field
The Paragraph field provides a text area that allows your visitors to enter a text-based message.
> Number Field
The Number field is formatted to only accept numeric characters.
> Date Field
The Date field allows you to select from a variety for data formats and ensures that the entry is stored in date field type. When leads select a date, a popup calendar will allow them to seamlessly select a date.
> Country Field
The Country field provides a drop-menu of every Country in the world.
> Mobile Phone
The Mobile Phone field was specifically created to store the value in a format that could be utilized for text message campaigns and workflow. There is an option to include country code.
> Phone Field
The Phone field is formatted for proper entry no matter what Country your visitor is from and is stored numerically. There is an option to include country code.
> URL Field
The URL field is formatted to accept web addresses.
> Terms and Agreement Field
The Terms and Agreement field allows you to enter a text-based agreement that a user must click to accept, before submitting your form.
> Drop Menu Field
The Drop Menu field allows you to create a multiple-option drop menu.
> Check-box List Field
The Check-box List field allows you to create a multiple-option check-list for users to check off accordingly.
> Radio Button Field
The Radio Button field allows you to create a multiple-option menu for multiple choice questions.
The Hidden field is a field that lies behind your landing page. Adding a default value and mapping it to a field in SugarCRM will automatically populate that field when a lead converts.
>U.S State Field
Pretty self explanatory :). Users can select what U.S state from a drop-down menu.
